Supporting HD loved one in move from low care to High Care Nursing Home

What are the main things to take into consideration when your loved one's current Low Care home has suggested it is time for them to be moved onto somewhere more high care/suited for HD? Specifically a younger person. 

My number one is to get in contact with Huntington's Victoria and speak to the guys there, but as a family member/carer/friend, what would be some advice you would give in this situation?

I would first discuss with the person as to the possibility of seeking more supports which may include appropriate accommodation. Assuming the person is on an NDIS plan, I would be exploring the possibility, if they qualify, in applying for an SDA property. (Supported Disabilty Accommodation). These are very limited and may not necessary be available in the preferred areas. It may be sharing with at least 1 other person but they are able to have their own support workers (hours depends on NDIS funding) I would also ensure that the support workers are adequately trained and aware of the needs of the person involved, including the person in these discussions. I have accessed Azena from HD Vic to co-ordinate specific training via zoom for the support workers which has been very beneficial
